Job Title: Care Coordinator

Locations: Akute - Ogun, and Ikeja - Lagos

Employment Type: Full-time Summary

We are looking for a dedicated Care Coordinator to assist us with patients at our facility.

Responsibilities

Develop and coordinate healthcare programs Communicate between staff, patients and family Handle patient case management and education Recruit and train staff and create schedules Develop patient goals and monitor progress Maintain patient confidentiality and quality care.

Requirements and Skills

Highly organized with a strong ability to manage different priorities Ability to work with others and use resources to problem solve Apply good judgment to fast-changing situations Strong customer service skills Effective verbal and written communication skills.

Salary
